All 16 and 17-year-old students must have a signed parental consent form/permission slip to donate.
Please fill out completely in ink.
(Nothing in pencil will be accepted)
You must be at least 16 years old and weigh 110 lbs. to donate.
Bring a valid I.D. Eat and hydrate prior to donating.
