BLOOD DRIVE

Friday, October 4th 

9:00am to 2:00pm

Rooms 104 & 105

Please sign-up for an appointment time to donate with Mr. Blanton in office 145 or click on this Sign up Link

All 16 and 17-year-old students must have a signed parental consent form/permission form printed on both sides of the same sheet to donate. 

Please fill out completely in ink. (Nothing in pencil will be accepted)

You must be at least 16 years old and weigh 110 lbs. to donate.

Bring a valid picture I.D.

Eat and hydrate prior to donating.

Remember every donor helps to save 3 lives.
